  2. Ration Card / Smart Card issued in the name of Parents (having name of child)
  3. Domicile Certificate of Child or Parents
  4. Electricity Bill / MTNL Bill/ Water Bill of the month of Nov / Dec 2017
  • Medical Fitness and Immunization Record of the child
  • Proof of Sibling (Real Sister) studying in this school only (if applicable) – Fee Receipt of the sibling shall be attached with
  • Proof of Single Girl Child (if applicable) – Affidavit signed by 1st class Magistrate
  • 2 passport size photographs of the Child along with 2 photographs of each of the parent
